- Current DACA work authorization card (Expiring 09/05/17 through 03/05/18)
- Copy of previously filed DACA application and USCIS receipt notices
- Passport (if you have one)
- Social Security Card (if you have one)
- 2 Passport-style photos
- Money order for $495 made out to “U.S. Department of Homeland Security”
